Abstract | Leprosy is a treatable illness that has been eradicated from several nations, and this is possible because of the availability of excellent drugs that target leprosy. Leprosy treatment evolved through time, beginning with chaulmoogra oil in 1915, then to dapsone monotherapy in 1946, and eventually switching to multidrug therapy in 1982. There have been reports of resistance to all first-line medications in the past 20 years, necessitating the use of newer, highly bactericidal medicines and their combinations in clinical trials against Mycobacterium leprae. This chapter describes the historical perspectives, global incidences, pathophysiology, eradication of leprosy, and recent progress in leprosy treatment, along with the drug classification, synthesis, mechanism of action, metabolism, structure-activity relationships, and adverse drug reactions of antileprotic drugs, followed by the recent advances in antileprotic drug discovery, drug resistance, and its management through newer targets against leprosy. The clinical trial status of different antileprotic agents has also been summarized. |
